Mutt Madness Rules and Prizes

The Mutt Madness contest presented by PetFirst Healthcare begins at 12:00 a.m. EST on January 7, 2008 and ends at 11:59 p.m. EST on April 7, 2008.
The contest is open to the general public as well as any current PetFirst Healthcare policyholders. Employees of PetFirst Healthcare, promotional agencies and their respective affiliates and associates and such employees' immediate family members and persons with whom such employees are domiciled are excluded from this contest.
All entries must be original and submitted via upload to the YouTube website during the contest period. To enter, submit/upload a video recording (in WMV, MOV or AVI format, maximum file size of 100 MB) of a pet video.
PetFirst Healthcare must be notified of the YouTube link where the video is saved during the contest period.
One Champion, 3 Furry Four Winners, 4 Energetic Eight Winners and 8 Shaggy Sixteen Winners will be chosen by the online community, via a provided voting/selection system, with final approval by PetFirst Healthcare. All decisions by PetFirst Healthcare are final.
Starting at midnight, January 7, 2008, anyone can go to a video entry page, review the video, and select their favorite video. Voting will end on 11:59 p.m. EST on March 15, 2008.
Online voter selections will dictate which videos move to the “Shaggy Sixteen”.
Starting at midnight, March 16, 2008, “Shaggy Sixteen” finalists will be posted. Anyone can go to a video entry and, again, select their favorite video.
Online voter selections will dictate which videos move to the “Energetic Eight”.
Starting at midnight, March 24, 2008, “Energetic Eight” finalists will be posted. Anyone can go to a video entry and, again, select their favorite video.
Online voter selections will dictate which videos move to the “Furry Four”.
Starting at midnight, March 31, 2008, “Furry Four” finalists will be posted. Anyone can go to a video entry and, again, select their favorite video.
Each online visitor can only select a video once during each contest period (First Round, Shaggy Sixteen, Energetic Eight, Furry Four). If an online visitor changes their mind about the rating that they have given to an entry during a contest period, the most recent rating selected will be calculated into the total, and the previous selection will be removed.
The Champion will receive $1,000 and a year of Basic pet insurance for a dog or a cat.
The Furry Four winners will receive $500.
The Energetic Eight winners will receive $200.
The Shaggy Sixteen winners will receive $100.
Inappropriate or objectionable content, as determined by PetFirst, will not be eligible for voting or winning contest prizes.
All taxes and any other costs and expenses not specifically stated within these official rules are the sole responsibility of the winners. Prizes may not be transferred or substituted, except at PetFirst’s sole discretion.
The selected winners will be notified by email or telephone within twenty-four (24) hours of the close of the contest period. The potential winners will have ninety-six (96) hours from the moment such notification is issued to respond to such notification by following the instructions provided therein. If the potential winner does not reply to the notification within ninety-six (96) hours of its issuance or he/she cannot or does not comply with these official rules, he/she will be disqualified, the prize will be forfeited, and an alternate winner may be selected at PetFirst’s discretion.
Decisions of the contest judges are final—no substitutions will be available.
By claiming the prize, the winner authorizes the use, without additional compensation, of his or her name and/or likeness and/or voice/photograph and municipality of residence for promotion and/or advertising purposes in any manner and in any medium (including without limitation, newspapers and other publications, slides and distribution over the Internet) which PetFirst may deem appropriate.
This contest is subject to all federal, provincial and municipal laws.
